	Boys Pitching Log 2012 Sample Pitching Log	Little League has implemented new pitching rules primarily for the health and safety of the pitchers involved. Please make sure you understand and follow these rules. Any coach who knowingly violates the rules pertaining to number of innings pitched per week or rest between games will be subject to disciplinary action by the NCLL Board of Directors. This log is used to track pitches and needs to be available and signed at each game played.

	Volunteer Form 2012	Managers, coaches, board members and any other persons, volunteers or hired workers, who provide regular services to the league and/or have repetitive access to or contact with players or teams must fill out the Volunteer Application form as well as provide a government-issued photo identification card for ID verification.
	Medical Release Form 2012- AllStars	Medical Release form require by Little League to participate in District Tournament.
